Your workplace includes 3-5 Tertianum facilities in the Zurich region, according to agreement. The working days are Monday to Friday.

We are looking for a

Specialist Responsibility Spitex for interRAI-HC Assessments 90% -100 %

Your tasks

• Responsible for timely and correct planning and execution of initial assessments and reassessments (needs assessments) as well as needs notifications
• Planning of assignments (deployment plan) for the Spitex teams of the assigned facilities 
• Collaboration with health insurance companies -> processing referrals/feedback
• Professional and warm consultation and care of people in need of care
• Observation and analysis of care and support situations including initiation of necessary corrective measures
• Checking Spitex services for completeness and correctness for all clients (monthly closing)
• Coaching employees in the facility for correct and complete recording of Spitex services

Your profile

• Completed nursing training HF/FH/DNII or equivalent SRK-recognised qualification
• At least 2-3 years of professional experience in nursing
• 1-2 years of professional experience in Spitex with needs assessments
• Ability to analyse referrals
• Knowledge of correct planning and recording of Spitex services (including billing)
• Willingness to travel within the canton of Zurich
• Flawless German skills, differentiated oral and written expression
