What you will achieve with us

• Support of patients with various diseases along structured outpatient treatment pathways over 3-12 months between acute and general practitioner care
• Outcome-based care with standardised success monitoring as well as multiprofessional assessments to ensure holistic treatment
• Close interprofessional collaboration with acute care, therapists, nursing staff and consultants for optimal indication and follow-up treatment
• Management of outpatient internal medicine clinics as well as supervision and training of assistant physicians along a holistic patient care approach
• Co-design and further development of innovative projects in integrated care as well as taking responsibility in a dynamically growing area

What you bring with you

• Specialist in General Internal Medicine (FMH or equivalent recognition)
• Experience in general practitioner care, rehabilitation or cardiopulmonary performance diagnostics is an advantage
• Confident handling of somatic and psychological complexity
• Clear and appreciative communication with patients as well as within the interprofessional team
• Strong empathy and high social competence
