For a modern, non-profit organisation in Baselland, we are looking for a committed and competent HF nursing specialist with an interest in the focus area of dementia. Requirements Profile
- Diploma as HF nursing specialist
- Solid professional experience in long-term or home care nursing
- Further training in the field of dementia or willingness to obtain the corresponding qualification
- Independent, responsible way of working
- Strong social and professional competence as well as high empathy
- Driving licence category B and willingness to be mobile
Tasks
- Resource-appropriate care and support of people with dementia in their familiar home environment
- Planning, management and evaluation of the care process within the scope of case responsibility
- Conducting assessments and review meetings (RAI-HC)
- Supporting clients in everyday life to promote independence and quality of life
- Close cooperation with relatives, doctors and other specialist agencies
- Day shifts from Monday to Friday (including public holidays)
