Case Manager Psychiatry, Certified Nursing Specialist HF/FH (a)
No night shifts
What you will achieve with us
- Managing the care process for psychiatric clients
- Determining the service needs, creating the assistance and care plan
- Ensuring the professional provision of care services, guaranteeing differentiated, systemic and specialised care
- Taking on nursing assignments with our clients at home
- Carrying out interventions and maintaining electronic documentation
- Professional leadership of staff and coaching in complex care situations
What you bring
- Basic training as a nursing specialist HF/FH or equivalent with SRK recognition
- Several years of nursing professional experience - further training in psychiatric nursing or corresponding professional experience (at least 2 years)
- Competent assessment of nursing situations
- Flair and enjoyment of administrative and organisational tasks
- Good IT user skills
- Willingness to travel by bicycle
