About the job
The Réseau Santé la Côte (RSLC) is one of the four Vaud care networks defined in the cantonal law on care networks (LRS). It brings together regional care providers (hospitals, nursing homes, CMS, doctors, etc.) and municipalities. Its mission is to improve, in close collaboration with its members, the coordination of care and the guidance of users within the health system.
The RSLC manages BRIO, whose main missions are: guiding people with loss of autonomy upon hospital discharge, managing and evaluating requests for nursing home accommodation, and providing information on medico-social services to the public and professionals.
To complete its team, Réseau Santé la Côte is looking for
a coordination nurse
Activity rate: 50% - Permanent contract
Start date: 1 January 2027 or to be agreed
Within BRIO, your role:
- Respond to requests for nursing home accommodation and other structures within the system according to availability.
- Assess requests and propose solutions to applicants that match their needs and wishes.
- Participate in and organise network meetings when situations require it.
- Centralise and disseminate information on the services of health and medico-social institutions in the region and canton.
- Provide information, support, advice, and assistance to the public and network professionals.
- Participate in and represent, by delegation, Réseau Santé La Côte in working groups (for example, BRIO activity development projects).
Your profile:
- Bachelor’s degree in nursing at HES level or equivalent.
- Postgraduate certification in elderly care and support is welcome.
- Professional experience in elderly care.
- Good knowledge of the Vaud medico-social system and its institutions.
- Negotiation skills, listening ability, and empathy, especially for complex situations.
- Enjoy working in a team.
- Enjoy interacting with a large network of regional and cantonal partners.
- Experience or training in psychiatry is welcome.
- Proficiency in French (written and spoken).
- Vehicle and driving licence required.
