Your Tasks

• Independent and professional care and support of clients at home
• Planning, management and evaluation of the entire care process
• Implementation of demanding nursing and medical measures
• Recognising and assessing changes in health status and initiating appropriate measures
• Advising and supporting clients and their relatives
• Taking case responsibility and coordinating care
• Interdisciplinary collaboration with doctors, therapists, relatives and other specialist agencies
• Ensuring complete and professional nursing documentation
• Contributing to securing and further developing care quality
• Supporting and professionally guiding employees and trainees

Our Requirements

• Completed training as a Certified Nursing Specialist HF/FH or equivalent SRK recognition
• Ideally professional experience in Spitex or outpatient care
• High professional and social competence as well as pronounced sense of responsibility
• Independent, structured and solution-oriented working style
• Enjoyment in taking responsibility and interdisciplinary collaboration
• Empathetic and professional interaction with clients and relatives
• Willingness to work early and late shifts as well as weekend duties
• Good German language skills, spoken and written
• Driving licence category B advantageous or required depending on area of deployment
