We are looking for the CMS of Moudon a:
COMMUNITY HEALTH AND CARE ASSISTANT (ASSC) at 50%-80%
Mission:
- Collaborate on the medico-social care project (support in the form of help and care) based on respect for the principle of personal autonomy.
- Participate in monitoring the evolution of the client’s situation, in collaboration with the client, their entourage, the interdisciplinary team and the attending physicians.
- Provide clients with services such as basic care, nursing care, household help and family assistance.
Profile:
- Hold a Federal VET Diploma (CFC) as an ASSC.
- Have, if possible, experience in home care.
- Be available to work during the day, evenings and weekends.
- Be capable of working autonomously and interdisciplinarily.
- Hold a driving licence and own a vehicle (possibility to use a company vehicle).
