Les Peupliers provides 7 days a week replacements day, evening and night at the 7 CMS of the Fondation Soins Lausanne to ensure continuity of care for our clients.
To complete the team at the CMS des Peupliers, the Fondation Soins Lausanne is looking for immediately or as agreed three:
Coordinating Nurse (M/F)
Activity rate from 50% to 100%
Permanent contract
Your missions
- Identify and analyse the client’s immediate needs and establish the initial intervention plan to optimise continuity of care.
- Secure the client’s return home and coordinate care during the support period.
- Provide care during the support period.
- Ensure an on-call telephone service in the evenings, weekends and public holidays.
- Collaborate with the client network’s stakeholders and respond to their requests.
- Intervene if necessary in acute situations at home.
Your profile and skills
- Bachelor’s degree in nursing or equivalent qualification (foreign diploma recognised by the CRS)
- Experience in the medico-social field and interdisciplinary collaboration
- Knowledge of the care networks in the canton of Vaud
- Sense of responsibility, priorities and autonomy
- Driving licence essential and vehicle desirable
