Because every home intervention is unique, we are looking for autonomous, organised nurses with excellent interpersonal skills to support patients in their living environment.
For several of our partners active in home care throughout the canton of Vaud, we are recruiting nurses for short- and long-term temporary assignments. Interventions are distributed by geographical sector according to your place of residence and the needs of the teams.
Home Care Nurse
Your mission
Within multidisciplinary teams, you ensure the comprehensive care of patients at home in accordance with their care plan, promoting their autonomy and their ability to remain at home.
Your main responsibilities:
- Perform technical and relational nursing care for patients.
- Assess the health status of beneficiaries and ensure appropriate follow-up.
- Participate in the development and updating of the care plan.
- Collaborate with doctors, family caregivers and various health network stakeholders.
- Ensure communication and traceability of the services provided.
- Contribute to prevention, therapeutic education and the maintenance of patient autonomy.
- Participate in the continuity and quality of home care.
Your profile
- Bachelor HES-SO in nursing or foreign diploma recognised by the Swiss Red Cross.
- Professional experience in home care, medicine, geriatrics or hospital environment is an asset.
- Ability to work independently while integrating into a multidisciplinary team.
- Excellent interpersonal skills and organisational sense.
- Flexibility, adaptability and effective priority management.
- Driving licence essential.
- Personal vehicle, an asset depending on the intervention regions.
