Your Tasks
- Independent care and support of clients in their home environment
- Conducting needs assessments and planning individual care
- Supporting, advising, and providing professional assistance to caring relatives
- Ensuring and checking proper care by relatives
- Performing treatment care and other nursing measures
- Managing and documenting the care process
- Recognising changes in health status and initiating appropriate measures
- Interdisciplinary collaboration with doctors, therapists, and other specialists
- Quality assurance and complete nursing documentation
Our Requirements
- Completed training as a qualified nurse HF/FH or equivalent SRK recognition
- Experience in Spitex or outpatient care is an advantage
- Enjoy working with caring relatives
- Independent, structured, and responsible working style
- High social and advisory competence
- Good German language skills, spoken and written
- Driving licence category B and own car are mandatory
- Willingness to undertake mobile assignments at clients’ locations
