The position is attached to the Operations Directorate , which is tasked with providing optimal conditions for the delivery of medical and nursing services, ensuring the integration of the patient pathway in collaboration with medical, nursing and administrative partners, and connecting the patient pathway and clinical itinerary in partnership with the Medical Directorate and the Nursing Directorate. Its organisation is divided into 4 entities: the Patient Flow Management service, the receptions and telephone switchboard, the on-site patient Transport service, and the Social Service.
As a social worker within the HUG, you will work with patients in the geriatrics and rehabilitation department using a holistic and multidisciplinary approach. You will ensure the social follow-up of hospitalised patients, particularly in an acute care unit.
Your role consists of assessing the social needs of patients and their relatives, identifying social difficulties, and proposing appropriate solutions. You will be involved in managing administrative and financial requests and support patients in their efforts to remain at home, undergo rehabilitation, or transition to another facility (nursing homes, care structures). You will work closely with the internal multidisciplinary team as well as external partners.
Your work is based on individual interviews, family meetings, and multidisciplinary conferences to establish a relationship of trust with patients, understand their situation, and develop concrete and personalised solutions. You are also responsible for the rigorous communication of patients’ social needs and plans to the care team and external partners.
Finally, you actively participate in professional monitoring on social and legislative topics, particularly social insurance, and in the prevention of social problems. You are involved in the continuous improvement of practices within the service.
Qualifications
You hold a Bachelor of Science in Social Work or an equivalent qualification. Holding a CAS as a trainer practitioner and/or a Federal Certificate in social insurance is an advantage.
You possess solid knowledge of the social network in the Geneva area as well as social insurance. Experience in social work in a hospital setting and/or geriatrics is also an asset.
With excellent interpersonal skills, you enjoy working interdisciplinarily and are comfortable managing complex and emotionally demanding situations.
Organised, you know how to manage priorities, demonstrate autonomy in your work, and have excellent writing skills. You are also proficient with common computer tools.
