Case Manager Nursing, Dipl. Nursing Specialist HF/FH (a)
No night shifts
What you will achieve with us
- Managing the nursing process for somatic clients
- Determining the service needs, creating the assistance and care plan
- Ensuring the professional provision of nursing services, guaranteeing differentiated and specialised care as well as professional wound and pain management
- Carrying out nursing assignments at our clients’ homes
- Performing interventions and maintaining electronic documentation
- Professional leadership of staff and support in complex nursing situations
What you bring
- Basic training as a nursing specialist HF/FH or equivalent with SRK recognition
- Several years of professional nursing experience
- Competent assessment of nursing situations
- Flair and enjoyment of administrative and organisational tasks
- Good IT user skills
- Willingness to travel by bicycle
