Your tasks – our position
- Needs assessment in complex patient situations (assessments)
- Communication with patients and their relatives regarding the planning/organisation of follow-up solutions after hospital discharge
- Collaboration with Spitex organisations, rehabilitation clinics, nursing and care homes, and other institutions to manage and organise targeted aftercare for patients
- Obtaining cost approvals for inpatient aftercare
- Supporting care teams to maintain optimal length of stay by early clarification and management of inpatient progress and discharge planning
- Interdisciplinary collaboration with nursing wards, nursing experts, doctors, and clinic secretariats
Your skills – our requirements
- Completed training in the healthcare sector as a registered nurse HF/FH with professional experience in inpatient care
- Organisational talent, flexibility, resilience, and service orientation
- Good communication, assertiveness, and ability to think in networks
- Good IT user skills
- Communication in German or French (spoken and written) with good knowledge of the other language or willingness to learn it
- Committed personality with motivation and initiative to significantly contribute to the further development of our organisation in a changing environment
