The Medical-Social Centres have the mission to help people of all ages who are dependent or have health issues to remain in their living environment as long as possible and to support their caregivers. ASPMAD, with its 9 CMS and 950 employees, annually cares for 6,700 clients at home and serves 105 municipalities in northern Vaud.

Your duties

As a lead nurse, your role involves advanced mastery of specific skills:

- Personalised planning: Propose, organise and coordinate intervention plans tailored to clients, ensuring their satisfaction and respect for their needs.

- Rigorous follow-up: Supervise and regularly reassess situations, coordinate essential actions for safety and continuity of care.

- Document management: Responsible for managing OPAS forms and updating client files in accordance with guidelines.

- Interdisciplinary collaboration: Work closely with the entire network (CMS team, caregivers, doctors, partners) to ensure optimal care.

Your profile

You hold a HES nursing diploma or an equivalent qualification, supported by significant experience of 2 years, preferably in home care.

You are versatile, attentive and autonomous; possess excellent interpersonal and negotiation skills and show a strong interest in home care.

Your organisational skills, ability to anticipate needs and adapt to all types of situations, and competence in stress management are essential qualities.

You hold a driving licence and ideally have a personal vehicle.
