The aftercare and social counselling service at the Winterthur Cantonal Hospital is an independent specialist group that is part of the interdisciplinary treatment team and ensures integrated care with viable and sustainable aftercare solutions.
We are looking for reinforcement and look forward to your application as a Case Manager / Social Worker for Aftercare and Social Counselling.

Your tasks
- Co-responsibility for effective discharge management together with all professionals involved in care
- Playing a central role in aftercare and social counselling in the recovery process of patients
- Close collaboration with various professional groups within and outside the KSW
- Development and implementation of solutions for the best possible reintegration into society, family and working life
- Active networking and coordination with internal and external agencies
- Working in a respectful, positive environment with high recognition of your work
Your profile
- Completed training at a university of applied sciences for social work or further training in case management. A basic medical education rounds off your profile
- Initiative, flexible and resilient personality
- Enjoyment of interdisciplinary collaboration as well as cooperation with external counselling centres and institutions
- Strong communication skills and high commitment to optimal solutions for patients and relatives
- Very good counselling skills and confident handling of different interests and expectations
- Professional experience in an acute hospital, in inpatient rehabilitation or in a comparably complex environment
