Day Nurse (HES) at 80%
We are looking for a day nurse to strengthen our health services. Your mission is to ensure health responsiveness for several living groups. You guarantee the continuity and quality of nursing care, develop prevention and health promotion actions, and work in close interdisciplinary collaboration with socio-educational staff and network partners, respecting the institutional concept.
Mission:
Health responsiveness and continuity of care:
- Be the reference person for health matters for several living groups
- Ensure the continuity of preventive, curative, palliative and delegated nursing care
- Identify critical situations and respond appropriately
Prevention and health promotion:
- Implement prevention, health promotion and screening actions
- Promote hygiene measures and health-supportive behaviours
- Inform and raise awareness among teams and residents on health issues
Interdisciplinary work:
- Collaborate closely with socio-educational staff in support
- Participate in interprofessional meetings and contribute to joint reflections
- Work with external partners (doctors, hospital, CMS, etc.)
Monitoring and quality:
- Keep medical records and nursing documentation up to date
- Prepare health summaries and care plans.
Profile:
- HES or ES nursing diploma, or equivalent qualification
- Strong interest in prevention, health promotion and community health
- Experience in community health is an asset
- Ease in interdisciplinary work with socio-educational teams
- Sense of responsibility, autonomy and analytical skills
- Human, collaborative and solution-oriented approach.
